Note 01: Every applicant must upload a photo.
Note 02: An application is treated as complete only once every section has been filled in.
Note 03: Only one application per applicant is accepted for the tablet assistance. Submitting more than one application, whether from a single institute or across multiple institutions, will lead to rejection of the applications by the competent authority, with no exceptions entertained.
Note 04: Providing wrong or misleading information in the application will result in its rejection by the Department authorities.
Note 05: All required documents must be scanned carefully, following the given instructions.
Note 06: Applicants should fill out the form carefully, since the institution cannot be changed once the application has been successfully submitted.
Note 07: Uploaded documents that are unreadable give the competent authority grounds to reject the application.
Note 08: Students studying within the state cannot submit this application form directly and individually; only students studying outside the state need to fill it out online and submit it to their native district officer.
