About Support for Marginalized Individuals for Livelihood and Enterprise (SMILE): Composite Medical Health for Transgender Persons
Rolled out on 12th February 2022, Composite Medical Health for Transgender Persons forms part of the “Comprehensive Rehabilitation for Welfare of Transgender Persons” sub-scheme under the Ministry of Social Justice and Empowerment’s “Support for Marginalized Individuals for Livelihood and Enterprise (SMILE)” scheme. It aims to give every transgender person living in India health insurance coverage so they can access proper treatment, including sex reassignment surgery, and other medical support to improve their health. The health cover comes as Ayushman Bharat Plus, encompassing gender reaffirmation surgery and comprehensive transition-related healthcare — including hormone therapy and post-operative care — redeemable at both private and government healthcare facilities.

The applicant must hold the Transgender Certificate issued by the "National Portal for Transgender Persons", as this is mandatory to avail the health insurance package.
Beneficiaries who already have their certificate from the National Portal are automatically eligible for the insurance.

The transgender beneficiary/applicant holding the Transgender Certificate should visit an empanelled hospital or a Common Service Centre (CSE).
These empanelled hospitals are drawn from the list of institutions already approved under the Ayushman Bharat Scheme.

Verification of Certificate Details on the System
The applicant must present their registration Identification/Transgender person's certificate serial number from the National Portal to the facility/centre staff.
The beneficiary's details are then entered and verified in real time, following the Beneficiary Identification System procedure used for Pradhan Mantri Jan Arogya Yojana beneficiaries.

Anyone without the mandatory Transgender Certificate is redirected to the National Portal website to obtain it.
Once the Transgender Certificate is verified successfully, the eligible applicant is issued the specific Ayushman Transgender Health card.
